  • Health and Social Services Home Care Program

    The Program’s objective is to support individuals so that they can live independently in their homes The Program is a territory-wide program of the Government of Yukon. It is administered by the Continuing Care Division under the Department of Health and Social Services.

    Types of support they provide:

    Social Workers assist in identifying client needs and supports. They provide advocacy, counselling and support in accessing community resources.

    Home Support Workers assist people with bathing, grooming, light housekeeping, laundry, and grocery shopping and meal preparation.

    Occupational Therapists help people who have had a change in their ability to perform daily activities such as getting dressed, bathing, and leisure activities. They make recommendations regarding home safety adaptations and mobility aides.

     Physiotherapists assist people with physical challenges to become more mobile, physically fit and independent.

    Nurses provide education and treatment related to medical conditions, and liaison with the medical community.

    Speech Language Pathologists provide assessment and consultation for individuals who have difficulty communicating and swallowing.

  • Continuing Care Residential Care Programs

    Located at 109 Copper Road in the Industrial area  in Whitehorse

    Continuing Care provides respite, day programs and permanent placement for people with Dementia.  They have three locations at which they provide these services, Macaulay Lodge in Riverdale, Thompson Center next to Whitehorse General Hospital and Copper Ridge Place in Grainger.

  • Social Worker/Counselling Assistance

    These workers provide assistance and aid for families or spouses caring for someone with Dementia.  If your loved one is in care at one of the facilities they are available for you for support and counselling.

    If you care caring for your loved one at home you can still contact them for help and they will provide support and point you in the right direction
